Patient Billing

We provide exceptional care at the most practical cost to our patients, therefore at the time service is provided, payment is due unless another agreement was made prior. If you have questions regarding treatment or your account, please contact us at (480) 994-9494. Many times, a simple telephone call will clear any misunderstandings.

Irrespective of insurance coverage, please remember you are subject to all fees charged by our practice.

A monthly statement will be sent to you. Within 4-6 weeks most insurance companies will reply. If your statement does not match your insurance payment, please call our office. You are responsible for any remaining balance after your insurance has paid. Your prompt remittance is appreciated. Arrangements can be made for a monthly payment plan but this must be done preceding the actual procedure.
